Participant Release and Waiver of Liability

I, the undersigned parent or guardian, hereby give permission for all above-named athletes to participate in all Cultural Wealth Sports Group (CWSG) activities, including practices, games, clinics, and special events during Season 4 (November 2026 – January 2027).

I acknowledge and understand that participation in sports activities involves inherent risks of injury. I hereby release and discharge CWSG, its commissioners, coaches, volunteers, and affiliated organizations from any and all claims, demands, or causes of action arising out of or relating to any injury, accident, or illness that may occur during participation.

I authorize CWSG to provide or obtain emergency medical treatment if necessary, and agree to be financially responsible for any medical expenses incurred. I grant CWSG permission to use photographs and videos of my children for promotional and social media purposes. I confirm that all information provided in this registration is accurate and complete.

Season 4 dates: November 7, 2026 – January 10, 2027. Opening Day: November 7, 2026. Championship Day: January 10, 2027.
